Overview
Multiple myeloma is a malignancy of terminally differentiated plasma cells that accumulate clonally in the bone marrow, producing a monoclonal immunoglobulin or light chain and disrupting normal haematopoiesis. It characteristically gives rise to the constellation of hypercalcaemia, renal impairment, anaemia, and lytic bone disease, together with increased susceptibility to infection, and it is typically preceded by the premalignant states of monoclonal gammopathy of undetermined significance and smouldering myeloma. Diagnosis integrates serum and urine protein studies, marrow examination, imaging, and cytogenetics, while treatment combines immunomodulatory drugs, proteasome inhibitors, corticosteroids, monoclonal antibodies, and autologous stem-cell transplantation.
