Overview

Bladder cancer is a malignancy arising from the tissues of the urinary bladder, most commonly the urothelium that lines the bladder wall. The predominant histological type is urothelial (transitional cell) carcinoma, with squamous cell carcinoma and adenocarcinoma occurring less frequently. The hallmark presenting feature is painless hematuria, sometimes accompanied by urinary frequency, urgency, or pain on voiding. A central clinical distinction is between non-muscle-invasive disease, confined to the mucosa and lamina propria, and muscle-invasive disease that penetrates the detrusor muscle, since this separation governs prognosis and treatment. Established risk factors include tobacco smoking and occupational exposure to aromatic amines and other industrial chemicals, while chronic irritation and certain infections contribute in some settings. Diagnosis relies on cystoscopy, urine cytology, imaging, and transurethral resection with histopathological staging and grading. Management is stage-dependent and may combine transurethral resection, intravesical therapy, radical cystectomy with urinary diversion, systemic chemotherapy, radiotherapy, and immunotherapy. Surgical reconstruction such as ileal conduit diversion can itself give rise to later complications. Because non-muscle-invasive tumors recur frequently, long-term surveillance is integral to care, and ongoing research addresses molecular characterization, biomarkers, and bladder-preserving strategies.
