Overview

Rheumatoid arthritis (RA) is a chronic, systemic autoimmune disease characterized by symmetric inflammatory polyarthritis that targets the synovial joints. The hallmark lesion is synovitis, in which an inflamed, hyperplastic synovium (pannus) invades and erodes articular cartilage and subchondral bone, producing pain, swelling, morning stiffness, and progressive joint deformity. Disease pathogenesis involves dysregulated innate and adaptive immunity, with autoantibodies such as rheumatoid factor and anti-citrullinated protein antibodies, activation of T and B lymphocytes, and pro-inflammatory cytokine networks (TNF-alpha, IL-6) that recruit and sustain effectors including increased synovial megakaryocytes. Genetic susceptibility, exemplified by transcription-factor and signaling polymorphisms such as STAT4, interacts with environmental triggers to break immune tolerance. Beyond the joints, RA carries systemic manifestations and comorbidity, and oxidative stress and antioxidant balance are relevant to tissue injury. Management has shifted toward early diagnosis and treat-to-target strategies using conventional and biologic disease-modifying antirheumatic drugs, with emerging interest in precision rheumatology to tailor therapy; adverse effects of agents such as leflunomide are also clinically important. Related inflammatory and septic arthritides inform differential diagnosis. The journal publishes peer-reviewed clinical, immunological, and translational research relevant to autoimmune and inflammatory joint disease, its mechanisms, and its treatment.
