Overview

Infertility is the inability to achieve a clinical pregnancy after a defined period of regular unprotected intercourse, or the inability to carry a pregnancy to term, affecting both female and male reproductive function. Its causes are multifactorial and include hormonal and endocrine disturbances, ovulatory disorders, structural abnormalities of the reproductive tract, gamete quality deficits, immunological factors, lifestyle and metabolic influences, and advancing reproductive age; in a substantial subset the cause remains unexplained despite investigation. Evaluation typically encompasses ovarian reserve and endocrine testing, semen analysis and testicular assessment, imaging, and increasingly the search for biomarkers, while management ranges from ovulation induction and pharmacologic therapy to assisted reproductive technologies such as in vitro fertilization. Research reflected here spans immunological biomarkers in unexplained female infertility, morphological evaluation of testicular biopsies in infertile men, regenerative approaches using mesenchymal stem cells in premature ovarian insufficiency and ovarian aging, oxidative-stress and mitochondrial contributions to sperm quality, the reproductive and quality-of-life burden of endometriosis, and the metabolic and endocrine dimensions of polycystic ovary syndrome. Infertility matters because it carries significant physical, psychological, and social consequences and is a major focus of reproductive medicine and women's health. The journal publishes peer-reviewed research on the causes, diagnosis, biomarkers, and treatment of infertility.
